Session Pack Terms & Conditions

Overview

Welcome to Active Health Clinic! These Terms & Conditions ("Terms") govern your purchase and use of program and session packs ("Packs") offered by Active Health Clinic.
 

Throughout the site, the terms “we”, “us” and “our” refer to Active Health Clinic. Active Health Clinic offers this website, including all information, tools and services available from this site to you, the user, conditioned upon your acceptance of all terms, conditions, policies and notices stated here.

 

Please read these Terms and Conditions carefully before purchasing a session pack. By purchasing a session pack, you agree to be bound by these Terms and Conditions.

 

SECTION 1 - Pack Options and Validity Period

Active Health offers Packs containing either five (5) or three (3). The three (3) session Pack is only available for purchase after completion of the initial program, which includes an initial appointment and five (5) one-on-one sessions. The validity period for each Pack is as follows:

  • 5-Session Pack: Valid for six (6) months from the date of purchase.
  • 3-Session Pack (Only available once 6 sessions are completed): Valid for twelve (12) months from the date of purchase.

SECTION 2 – Service Applicability

Packs are redeemable for any service offered by Active Health Clinic, subject to appointment availability.

SECTION 3 – Payment and Refunds

All Pack purchases are final and non-refundable.
You are solely responsible for any decline fees associated with your chosen payment method.

SECTION 4 – Cancellation Policy and Missed Appointments

Cancellations made within forty-eight (48) hours of your scheduled appointment or failure to arrive for your appointment will result in the forfeiture of one (1) session from your Pack.

SECTION 5 – Pack Expiration

Any unused sessions within an expired Pack will be forfeited.

SECTION 6 – Amendments to Terms & Conditions

Active Health Clinic reserves the right to modify these Terms at any time. We will undertake reasonable efforts to notify existing Pack holders of any material changes.

SECTION 7 – Governing Law

These Terms shall be governed by and construed in accordance with the laws of Victoria.

SECTION 8 – Acceptance

By purchasing a Pack, you acknowledge that you have read, understood, and agree to be bound by these Terms.
